Consent: I/we give permission for photographs of the registrant to be used in promotional materials, websites, newspaper articles, or other social media uses.

I/we certify that the registrant on this form is in good health and can vigorously participate in all physical activities. I/we understand that our child participates at his/her own risk and fully relieves the Duffy Basketball Camp, its owners, guests, volunteers and representatives from all claims pertaining to loss or damage to the camper’s personal property or injuries sustained while at the house league.

Payment Options: Payment is accepted through either cheque or email money transfer. The cost is $300 or $290 if you are registering more than one child. (If you have 3 children, for example, you would pay $290 for each child).

We are a peanut/tree nut-free camp please refrain from sending your child with peanut or nut products.
